Project FOCUS (For Our Children’s Ultimate Success), is a Full Service Partnership Wraparound program designed to serve culturally and linguistically isolated Asian and Pacific Islander individuals and families in Orange County. The community-based, client-centered program develops individualized care plans for children and transitional age youth (TAY), ages 0 through 25 years. Program participants have serious emotional disturbances (SED) or severe mental illness (SMI) and are in one or more of the following priority categories:
Homeless or at risk of homelessness
Multiple psychiatric hospitalizations
First psychotic episode
Uninsured and exiting SSA or probation system
Unserved or underserved due to linguistic or cultural isolation
Special needs and/or co-occurring disorders
Parents have serious mental illness
In danger of failing school
The dedication and diversity of the Project FOCUS team allows the program to support individuals and families utilizing a “whatever it takes” approach, and assists them to rebuild, reconnect and reach identified goals. In-home and community-based intensive case management services provide support and assistance in obtaining such things as benefits for low-income families, health insurance, housing placements, parent education, tutoring, mentoring, youth recreation and leadership development. Services are available in Vietnamese, Korean, Tagalog, Samoan, Cambodian, Chinese, Japanese, Hmong and other Asian and Pacific Islander languages.
